Two treatments, both for multiple things! I'm a troubled person...
Talk therapy: Approximately 6-7 years in total, spread out. It wasn't specifically for phobia for the first 5-6 but even when it was, it wasn't super helpful.
Zoloft: Now I'm on 50mg and it's SO MUCH better than before. For a good year I had practically no anxiety. Recently it's been getting worse, but nowhere near the levels of before. I also take hydroxazine (or however it's spelled) in bad cases (instant panic attack stopper and mild antiemetic to boot!)

I've been on Zoloft for a few years, and while I still have panic attacks and days when I feel like crap, I'd say the day-to-day anxiety and psychosomatic symptoms are usually much better than they were

I dont do CBT or therapy but I do see a psychiatrist. I've been on multiple SSRI's and wellbutrin but they never worked out. I also used to take xanax but I became addicted and started to abuse it, so i'm tapering. It helps but it's dangerous. The way i've learned to help myself was through exposure. I avoided websites/blogs that give warnings for v* in tv/films, starting going to bars/clubs more often (I was scared of people getting sick from drinking to much), and not trying to hold back v* if it needs to happen. Now, i'm only anxious about SV or FP because of traumatic past experiences, so i'm still working on overcoming those obsessions. However, expose does help tremendously, whether it's assisted by a therapist or just on your own i'd highly recommend it.

No formal treatment for me. I came really close when my phobia was at its worst but then I looked at how much it was going to cost and that motivated me to make sure I did everything I could on my own first. And fortunately that worked for me... I was able to make huge progress toward recovery without any professionals but just drawing support from others who were also recovering.
